Permission for my child to participate in field trips away from the MNjcc. I understand that if my child does not partake in these trips it will be my responsibility to find him/her alternate arrangements for that day.

I understand that the descriptions of programs are subject to change before and/or during the year without prior notice.

I authorize MNjcc staff to obtain such medical care as they may deem it necessary in the event of injury or otherwise. I agree to pay for all expenses incurred thereby.

The MNjcc is not responsible for any claims or proceedings made or brought in respect of any cost, losses, damage or injury arising by reason of the participation of the child listed below in daily activities, or by reason or the provision of medical care to them.
